Welcome to the Sight & Hearing Program. We proudly serve the communities of McMinnville, Lafayette, and Dayton. Currently, we are also extending our services to Amity, Sheridan, and Willamina. If you live outside these areas, please complete the GET HELP FORM at OLSHF.org, and we will connect you with your local Lions Club or the District 36-O Task Force to ensure you receive the support you need.

The McMinnville Lions Club partners with specific care providers to offer low or no-cost eye exams, eyeglasses, and hearing aids. Each individual can apply for either hearing or vision services during each fiscal year, beginning July 1. You will need to decide which service you need most before applying. Eye examinations can be granted every two years, while hearing aids are available no sooner than five years after your last set.

To apply, you will need to include documentation that proves your financial need. Acceptable documents include (please attach all that apply):

• Last two pay stubs, W-2, or tax return

• Welfare benefit documentation or unemployment eligibility letter

• Social Security or disability documentation

• Food stamp eligibility letter (please do not send a copy of the OR Trail card)

• A written statement from a trusted organization (such as YCCO, YCAP, DHS, or Gospel Rescue Mission) verifying your lack of income
